Last October I confirmed with Gina at QDS that QDS had fabricated my upper temporary denture. The doctor I am unfortunately having to sue as the "permanent" denture has been described as more like a toy, listed QDS as the July 2018 lab, not the October 2017 lab. I gave Gina my social security number and name and asked if she had records, and she said, yes, we made your temporary. She said no, we have nothing in July. I said would it surprise you to know you were the lab listed for the July denture and that my dentist said the July lab filed bankrtupcy? I had 3 times by email for records which are mine under HIPAA. I sent a hand delivered request also. I am now having to file coplaints with the government and Arizona Dental Board just to get MY records. Make sure all third party vendors are HIPAA complaint and that you deal with those who follow the rules, particularly HIPAA requests.
